Before filling in the form, you should be registered in the suomi.fi-service. In case you have not registered yourself then go to https://www.suomi.fi/frontpage and from there to the `identification´ section.
In case you have registered yourself before then you can proceed by filling in the form.
When you are submitting the form you will be identified through the suomi.fi –service either through using your online banking codes or your Mobile ID.
We use the suomi.fi service for identification. Please also remember to allow messaging in the suomi.fi service, so that we can respond to your request.
Please note that you cannot order both laboratory tests and imaging results with the same form. If you need both, please fill in a separate request for both.
